Steps to Take Care of a Newborn
It can be difficult for a new parent to know when a baby is sick. Medical professionals field so many questions from mothers of newborns that they often treat concerns as nothing more than just new-mama nerves. Mothers can begin to doubt whether or not to trust their instincts.
Fortunately, even new parents can objectively measure their baby’s health.
THE FIRST WEEK
A baby should nurse a minimum of eight times each day for a period of at least a quarter of an hour each session during the first week of life. You should be able to hear the baby swallowing milk. Baby’s first sticky-black stool – called meconium – should pass. The texture and color will change from a blackish green to a brownie batter consistency. By day 4 or 5 the color will be yellow. During the first two days of life, a child should begin to have wet diapers. By the end of this week, the frequency of wet diapers will increase to 2 or 3 per day.
An ill baby will show definite physical signs of his condition. Nursing for less than fifteen minutes fewer than 8 times per day, showing no desire to eat and having a sucking reflex that is week are all signs for a parent to be concerned. If a baby falls asleep before nursing for 15 minutes, makes a clicking sound when nursing, and has dimpled cheeks when sucking, these are also indications that there is some difficulty. When the baby’s diapers don’t show stools progressing as they should and you don’t see wet diapers within 48 hours after birth, there is something out of the ordinary. Such symptoms persisting for two days mean your child should be seen by a health care provider.
THE FIRST FOUR WEEKS
Signs of a healthy baby will be similar for the first month. Each day a healthy baby will nurse for eight sessions and produce 2-4 yellow bowel movements. Urine should be clear, not yellow, 6-8 times per day. Along with observing some milk and hearing your baby swallow during nursing, you should see a strengthening of the suck reflex. The baby should increase in weight and alertness.
However, in this 2nd to 4th week, you should make a note of anything that is out of the ordinary such as a baby not eating at least 8 times per day, infrequent or small stools, too few wet diapers, or if the urine becomes bright yellow. Your baby should gain weight and grow longer. You’ll know something is not right if your baby’s sucking becomes weak, you can’t hear swallowing at feedings, the baby becomes lethargic and slow to respond or stops sleeping between feedings. These behaviors for two consecutive days mean you should seek medical assistance.
THE 5TH THROUGH THE 10TH WEEK
During this stage, the baby’s feedings may go down to seven per day. This is because he is growing and can take in more milk. Further changes may be noticed in bowel movements: they can either increase to several small stools per day, or one large one every third day. In breastfed babies, this is normal because the body assimilates much of the milk with little waste. The wet diapers (6-8 per day) will continue in frequency, but check to be sure the urine isn’t a yellow concentrate. You should notice that your baby is increasing in his awareness, has a stronger suck, audible swallowing and a little milk dripping at feeding time.
During this 5th to 10th week you may have reason for concern if your baby doesn’t nurse at least seven times per day or produce the right amount of wet (not bright yellow) diapers each day. Make a note if your child stops gaining weight or doesn’t grow in length. Weak, tired sucking without an audible swallow indicates the baby is not getting proper nourishment. Seek medical assistance if your baby becomes lethargic, slow to respond or unable to sleep between nursing sessions. Two consecutive days of these behaviors indicate something is wrong.
MONITORING YOUR BABY
Knowing when your baby last ate and how often can be complicated enough if you were operating on a full night’s rest. Asking this of a post-natal mama can be daunting. One simple way to remedy this is to leave a notebook and pencil near the baby’s sleeping area. Whenever mama feeds or changes the baby, note the time and what was done along with your observations. An example might be: “2:00 a.m., Nursed 20 minutes, wet diaper/clear, bm – greenish yellow.” For clarity, you may want to start a new page each day with the date written at the top.
Should you need to confer with your doctor, you’ll be able to reference your notes for detailed information. This is also a good bonding tool for a new mama. Your weariness may make it difficult for your to identify the cause of your baby’s crying, but your list will provide you with the information you need to know if it’s been too long between feedings or bowel movements – identifying the cry as hunger or constipation. This helps mama learn the different sounds of her baby’s cries. This goes a LONG ways towards helping a new mama learn to trust her instincts when it comes to her baby.
However, older mamas give sage advice when it comes to newborns and new mamas. Enjoy these first few weeks to their fullest. Don’t worry about filling your day with anything more than getting to know this new little one. They aren’t little for long.
